HIPAA Notice of Privacy Practices (Summary)
As a health care provider, Dr. Sajid Lopez is required by the Health Insurance Portability and Accountability Act (HIPAA) and applicable Florida law to protect the privacy of your protected health information (PHI) and to provide you with notice of our legal duties and privacy practices. The summary below describes, in general terms, how your information may be used and your rights. The complete Notice is provided at your first appointment.
How your health information may be used and disclosed
- Treatment: To provide and coordinate your care — for example, consulting or sharing records with other clinicians involved in your treatment, with your authorization where required.
- Payment: Because the practice is out-of-network, we generally provide you with a superbill so you may seek reimbursement from your insurer; any submission to insurance is initiated by you.
- Health care operations: For internal activities such as quality, scheduling, and administration.
- As required or permitted by law: Certain limited disclosures are required by law, including suspected child, dependent-adult, or elder abuse; a serious and imminent threat of harm to you or others; and valid legal or public-health requirements.
Other uses and disclosures — including most uses of psychotherapy notes and any marketing or sale of information — require your written authorization, which you may revoke at any time.
Your rights regarding your health information
- Request to inspect and receive a copy of your records.
- Request a correction or amendment to your records.
- Request confidential communications (for example, by a specific phone number).
- Request restrictions on certain uses and disclosures.
- Receive an accounting of certain disclosures.
- Receive a paper copy of the full Notice of Privacy Practices on request.
- File a complaint if you believe your privacy rights have been violated — with the practice and/or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
Telehealth
Telehealth (video) visits are conducted using a secure, HIPAA-aware platform. As with any technology, please join from a private location to help protect your own confidentiality.
Changes to this notice
We may update this Notice and will post the current version here with a revised "last updated" date. Material changes will be reflected in the full Notice provided at the office.
